* Provides short-term care and long-term rehab care for those recovering from injury, illness or surgery. Services include:

Physical, occupational and speech therapy services.
Wound treatment, pain management, and cardiac therapies.
Activities of daily living assistance and treatment.
Subacute care including but not limited to diabetes management, comfort and palliative care, and respiratory management.

Eligibility: Anyone recovering from injury, illness, or surgery. Call for more information.

Application Process: Must complete a state-required assessment called a Patient Review Instrument (PRI) and application. Both must be approved for admission. Call for more information.

Fees: Fees may vary. Medicare, Medicaid, private insurance and private pay accepted.
