* Provides permanent supportive housing tailored to the needs and goals of each resident.

Eligibility: Adults 55 years or older, homeless, Medicaid eligible, and needs assistance with at least two activities of daily living.

Application Process: Call for information.

Required Documentation: Call for information.

Fees: 30% of resident's income.
